A Mother’s Second Chance at Life: How Teamwork Saved Two Precious Lives

At just 29 years old, Zeenat was eagerly awaiting the birth of her seventh child when her pregnancy suddenly turned into a life-threatening emergency. At approximately 32 weeks of pregnancy, she arrived at the Obstetrics & Gynaecology Department of Dr. Sikandar Ali Mandhro Civil Hospital, Badin (Indus Hospital & Health Network), in critical condition. She was struggling to breathe, her blood pressure had reached dangerously high levels, her oxygen saturation had dropped significantly, and she was suffering from severe preeclampsia, a serious pregnancy complication that can rapidly become fatal for both mother and baby.

Adding to the complexity, Zeenat had underlying heart disease, including moderate mitral regurgitation (MR), moderate tricuspid regurgitation (TR), and pulmonary arterial hypertension, making her pregnancy exceptionally high risk. Recognizing the severity of her condition, a multidisciplinary team from the Departments of Obstetrics & Gynaecology, Anaesthesia, Medicine, and Paediatrics was immediately mobilized. Emergency treatment commenced without delay, including high-flow oxygen, nebulization, magnesium sulfate therapy, intravenous antihypertensive medication, and continuous monitoring. Simultaneously, arrangements were made to transfer her to a tertiary care facility with advanced cardiac and critical care services. However, despite every effort, her condition deteriorated rapidly.

Moments later, Zeenat suffered a cardiorespiratory arrest.

A Code Blue was activated immediately, and cardiopulmonary resuscitation (CPR) was initiated in accordance with Advanced Cardiac Life Support (ACLS) guidelines. Every member of the multidisciplinary team worked in seamless coordination, fully aware that every second was critical.

Following internationally accepted maternal resuscitation guidelines, Consultant Obstetrician & Gynaecologist Dr. Tania led the Obstetrics & Gynaecology team in making the critical decision to perform an emergency bedside perimortem cesarean section within four minutes of the cardiac arrest. As the patient was undergoing active resuscitation, transferring her to the operating theatre was not possible. The life-saving procedure was therefore successfully performed at the bedside.

A live baby was delivered and immediately handed over to the Paediatrics team, who promptly initiated neonatal resuscitation and stabilization. Resuscitation efforts for the mother continued relentlessly, and soon afterward, return of spontaneous circulation (ROSC) was successfully achieved, restoring Zeenat’s heartbeat.

Another challenge arose when no Intensive Care Unit (ICU) bed was immediately available. Through exceptional coordination and swift decision-making, a recovering ICU patient was safely stepped down, allowing Zeenat to be transferred without delay for ongoing critical care. Her recovery over the following days was remarkable. She was successfully extubated the next day, maintained stable oxygen saturation on room air, remained hemodynamically stable, and continued receiving comprehensive multidisciplinary care. As her condition steadily improved, she was transferred from the ICU to the High Dependency Unit (HDU) and later to the postnatal ward.

After making a satisfactory recovery, Zeenat was discharged home in stable condition.

This extraordinary case highlights the life-saving impact of rapid clinical decision-making, adherence to evidence-based emergency protocols, and seamless multidisciplinary collaboration. The timely leadership of Dr. Tania, together with the unwavering dedication of the Anaesthesia, Medicine, Paediatrics, ICU, Nursing, and support teams, transformed an almost impossible situation into a story of hope and survival.

Alhamdulillah, both mother and baby survived. Their recovery stands as a powerful testament to the expertise, compassion, and commitment of the multidisciplinary team at Indus Hospital & Health Network, Badin, where every possible effort is made to save every precious life.
